Corrections and Listing Concerns

How to report an inaccurate provider listing, claim your profile, request removal of an unclaimed listing, or send editorial corrections to HometownMind.

Tell us what is wrong

Please report inaccurate names, locations, provider types, profile details, offensive material, impersonation, or a provider who no longer practices at a listed location. Do not include sensitive health information in a directory correction request.

To report a provider listing, search the directory, open the provider profile, and use the profile's “Share a concern about this listing” dialog. The form sends the report to our team for review.

Provider owners: claim and manage your profile

If you are the provider or authorized practice representative, use the provider dashboard claim flow to identify the listing you control and manage the information shown on it. A claim connects an owner account to a profile; the public Verified provider badge additionally requires both identity and phone verification.

If the listing is not yours, do not claim it. Use the profile concern dialog or contact support so the team can investigate the record.

Request removal of an unclaimed listing

Each unclaimed profile includes a “Request removal” link. Submitting that form removes the unclaimed public-record listing from directory search, profile pages, matching, and provider sitemaps, and prevents a later NPPES import from recreating it.

The public removal form does not remove a claimed listing. A claimed listing is managed by its owner in the provider portal; if you are the owner, sign in to manage it. A listing that has already been removed may return a not-found response.
