Provider Data and Verification
How HometownMind obtains provider listing data, separates NPPES records from claimed profiles, and describes identity and phone verification.
Where provider listing data comes from
Many HometownMind directory listings begin with records from the National Plan and Provider Enumeration System (NPPES), the public NPI registry administered by the Centers for Medicare & Medicaid Services (CMS). NPPES records can include an NPI, provider or organization name, taxonomy, practice address, and license information reported in the registry.
A listing is a directory record, not a credentialing decision. The presence of an NPI or a taxonomy does not by itself establish that a person is currently practicing, accepting patients, affiliated with HometownMind, endorsed by HometownMind, or licensed in a particular state. Confirm important details with the provider and the relevant licensing authority before booking care.
NPPES license information is not a state license check
When a profile displays a license number as being on file with the federal NPPES registry, that wording describes exactly what we know: the number appeared in an NPPES taxonomy record. NPPES license fields are reported to CMS by the provider and are not a live lookup of a state licensing board.
HometownMind does not turn an NPPES license field into a claim that a license is active, unrestricted, or state-board verified. For a current license question, use the relevant state's official board or licensing lookup and ask the provider to confirm the credential.
Claimed, verified, and unclaimed listings
An unclaimed directory listing is a public-record listing that no provider account currently controls. A claimed profile means an owner account is connected to the listing and may add or manage profile information; claiming alone does not prove identity, licensure, insurance participation, availability, or clinical quality.
The HometownMind Verified provider badge has a narrow meaning: it refers to identity verification and phone verification. These are separate checks, and the badge should not be read as a state-board license check, a guarantee of availability, or a clinical endorsement.
Completeness, updates, and search visibility
A listing may be visible even when some details are missing. We use the information available, such as specialty, credential or title, profile details, a website, and provider-reported information, when deciding which pages to show in search engines. That search decision does not change what a visitor can see on a profile.
Provider-reported openings older than 14 days are not shown as current openings. We do not provide a universal last-verified date or guarantee when every listing field was last updated. Directory details can be incomplete or out of date, and an NPPES license number is registry information rather than proof of a current state-board license, so confirm important details directly.
